Criteria for referral:

Your patient must be registered with a practice belonging to the MHN, have Diabetes and at least one of the following;

Neuropathy
Peripheral Vascular Disease
History of an ulcer or amputation

You can request one of our Podiatrists to visit your practice and provide training on how to perform a foot health check for a person with diabetes. This is designed to complete the foot check component of of the Diabetes Annual Review and to help idendify at risk feet before problems occur. Make a request here.
